Care Conference
Care Conference, also called a family care conference, is a meeting where Fairvern Team Members (RPN, PSW, Manager of Support Services, Social Worker if applicable, Physio if applicable, and Activation) discuss a Resident's current health status, treatment plan, and future care needs with the Resident and Family/Support person, allowing for open communication, questions, and collaborative decision-making regarding care.
Conferences are held six (6) weeks after a Resident has moved to Fairvern and annually thereafter, they typically take thirty (30) minutes. Additional conferences can be requested by the Resident/Family at any time, if necessary.
Key aspects of a care conference:
Introductions:
The meeting will start with introductions of all team members involved and a clear explanation of the purpose of the conference.
Medical overview:
Healthcare providers will present the patient's current medical condition, including diagnosis, symptoms, recent changes, and relevant test results.
Care plan discussion:
The team will review the current care plan, explaining the rationale behind treatments, medications, and therapies.
Goal setting:
The Resident, Family/Support person and healthcare team will discuss the goals of care, considering quality of life and desired level of intervention.
Decision-making:
Depending on the situation, the Family/Support person will be involved in making decisions regarding treatment options, including potential risks and benefits.
Question and answer session:
Ample time will be provided for Resident and family members to ask questions and express concerns regarding care.
Next steps and follow-up:
The team will outline the next steps in the care plan, including any necessary follow-up
Who might attend a care conference:
· Resident: When possible, the Resident should be present to participate in discussions about their care.
· Family members: Immediate family members, significant others, or designated caregivers
Healthcare team: Nurses, social workers, physiotherapy, nutrition services, and activity services
Important points to remember:
Come prepared:
Review any records or questions you may have beforehand.
Express your concerns:
Feel comfortable sharing any worries or questions you have about care.
Ask for clarification:
If anything is unclear, ask for further explanation from the healthcare team.
