Hello Families,

The quality and satisfaction of your experience in our home and your time with us is extremely important and the best way for us to understand what we can improve and what we are doing well is to hear directly from you.

This year’s survey is being conducted online as well as through hard copy available in the screening area. We kindly ask that you complete yours by December 9, 2022.

All survey responses and information collected is anonymous, will be kept confidential, and will only be used to improve the services we provide.

If you have an iPhone, iPad, or a newer Android phone, you can access the survey through the QR code below. Just open your camera app and put the QR code in front of the camera. Your phone will ask if you would like to go to the website to complete the survey.

We truly appreciate you taking the time to complete the survey and look forward to seeing your feedback and comments which we will use to identify opportunities for improvement across the entire organization including working with your home’s leadership team. Additionally, like last year, this year’s survey contains a comment section for your feedback on how we continue to manage the COVID-19 pandemic plus the opportunity to point out anyone you would like to recognize. We welcome all feedback.
