We truly appreciate you taking the time to complete the survey and look forward to seeing your feedback and comments which we will use to identify opportunities for improvement across the entire organization including working with your home’s leadership team. Additionally, like last year, this year’s survey contains a comment section for your feedback on how we continue to manage the COVID-19 pandemic plus the opportunity to point out anyone you would like to recognize. We welcome all feedback.

Your input is the best insight we can have to focus our efforts and make your experience even better.

The Activity Department will be assisting the Residents with the completion of their survey!

    Thank you!
